FIRST 20 PATIENTS

1. Complete the 1-20 forms with patient name, address, DOB and Phone number.
2. Fill in the number of pellets accordingly
3. Fax to the number on the form

1. Print the form (jump drive)
2. Hand write “Testosterone” in the appropriate fields
3. Sign the form
4. Make Copies
5. Complete as needed and fax to (817) 860-6083

RECEIVING PELLETS

– Orders placed by 2:00pm will be processed that same day. Depending on the size of the order, the order will be shipped ASAP.
– We TRY not to ship out pellets on a Friday
– Orders of $250 or more will get free shipping
– All orders are shipped with testing results showing sterility and potency of the pellets included
– UPS usually take 1-2 days for shipping but can be longer

TIPS…

– Pellets should be stored at room temperature 59-86 degrees F
– Pellets have a 1 year expiration date from the day of production
